First: an EOB is not a bill
After treatment, the insurance company mails an Explanation of Benefits (EOB). It usually says, in small print, 'This is not a bill.' It is a summary of what the provider charged, what insurance allowed, what insurance paid, and what may be left for you. Do not pay from an EOB.
